You usually assign an email server a static IP address. When you order a commercial type connection they give you an allocation range of static IP addresses. Then you can have the select few log into the smtp server at the IP address xxx.xxx.xxx.xxx with their user name and password.
If you run an email server from home and your ISP finds out you may loose your account permanently.
